Official Pyro bio states that his suit contains asbestos, and also that he attempts to communicate through wheezes, diagnosis: Asbestos poisoning.
Also, this reminds me a lot of Medical Maladies, it's a good animation and goes through the possible issues the mercs have, although most are mental, such as Pyro's schizophrenia and Medic's sadist tendencies.
This is great though, as much as regular injuries would be healed either by Medic or the Respawn, it's good to see them more human and see they do suffer consequences.
